A notification must be made in writing and sent to the Swedish Form office both digitally (bjorn.nordin@svenskform.se) and physically. The registration must be accompanied by a receipt showing that the registration fee of SEK 29,500 has been paid. Payment should be made to Föreningen Svensk Form BG 5350-8172.
The notification should also include any other information that may be relevant to the Board's assessment.
Examples of such information are
- the name of the author
- description of what in the appearance of the product the notifier considers to be distinctive
- when the product was created and the market to which it belongs
- the process of creation of the product
- one or more paper images
- one or more digital images
